When calling into the Insure Oklahoma helpline, please refrain from requesting a
particular representative to assist you. All our staffs are trained to answer any type of
questions. We can’t cater to one particular client due to high volume of calls and we want
all clients to have a fair opportunity when speaking with a representative.
According to our policy 317:35-10-26(b) (1) (A):
(i) The federal or state income tax form for the most recent year is used for
calculating the income only if it is representative of the individual’s current situation.
The individual’s business records beginning the month income became representative of
the individual’s current situation is used if the income tax information does not represent
the individual’s current situation.
Therefore, we will not be accepting 2009 taxes after April 18 - for enrollment purposes, to
verify current income, individuals must comply with Federal and State law by providing a
copy of the 2010 tax return.
NEW COST OF LIVING
There is a new cost of living with a new income guideline effective April 1, 2011. For
anyone who applies in March and qualifies will have an April effective date. Please visit

PROOF OF CITIZENSHIP FOR EMPLOYEES AND SPOUSES
Employer attestation forms can only be used for the employees. The form must be filled
out, signed and dated by the employer. The form is a legal document attesting that
all the employees are citizen of the United States or qualified alien under the Federal
Immigra¬tion and Nationality Act. The employees’ spouse will still need to provide one
item for proof of U.S. Citizenship and one item for proof of identity.
