2011 License Renewal
We made it through 2010
license renewal with some
minor hiccups, but overall
I think everyone has adjusted to the change in
the renewal expiration
date. We should be back
on track now for 2011
license renewal which
your license you were
granted for 2010 will expire June 30, 2010. You
will be required to submit
the full $225.00 renewal
fee, along with the renewal form, proof of continuing education and
malpractice as well.
If we receive your license
postmarked after the
30th of June 2010 you
will be required to pay
the late penalty fee of
$150.00 including your
renewal fee. Your license
will also be placed in a
lapsed status on July 1,
2010 until we receive all
fees and forms as required by the Oklahoma
Chiropractic Practice Act
Title 59 O.S. 161.11. We
also have available online
license renewal which is a
convenient way for you to
go online and renew without having to mail any
documents unless you
have not requested your
malpractice carrier to
email us your proof of
malpractice insurance.
We were made aware this
year that if you choose to
have your malpractice
insurance carrier to email
us your declaration page,
you need to request that
be done through them.
We will also accept them
via facsimile at (405) 524-
9542.
Please make sure that
you contact our office if

you have any questions
regarding your renewal
that is what we are here
for. Also please make
sure not to throw important documents away
from the board because it
could be your renewal
form!!
Continuing Education
If you have completed CE for your 2010 license prior to the granted extension of
December 31st, then you can take the last two remaining fall seminars for CE for
2011 if you want to go ahead and get them taken care of prior to the Spring of 2010.
However, it is my advice that you notify the state association that you intend to use
them for 2011 CE so that they will be made aware. If you do not attend the CEs
this fall for 2011 you will have one or two in the Springtime that you will be able to
attend for your continuing education hours. Remember if you do take any CEs out-
of-state and they have already been approved by the Board, you can only use 8
hours and it is your responsibility to provide that with your renewal form and fee.
