Programa de planificación familiar de Oklahoma para mujeres y hombres sin seguro médico que no son elegibles para sevicios de SoonerCare regular. SoonerPlan What is SoonerPlan? SoonerPlan is Oklahoma’s family planning program for women and men. Even people who qualify for SoonerCare may choose SoonerPlan instead. Who qualifies? U.S. citizens or qualified immigrants Oklahoma residents Applicants not enrolled in SoonerCare or Insure Oklahoma Applicants within the income standard (185 percent of Federal Poverty Level) Applicants with or without other insurance Women who have completed their pregnancy-related SoonerCare coverage. How do I apply? Enroll online at: www.mysoonercare.org. Visit your local health department. To find your local state/county/city health department office or clinic, call Oklahoma Area-wide Services Information System (OASIS) at 800-426-2747 from 8 a.m. to 6 p.m., Monday through Friday. This call is free. Visit your local county Oklahoma Department of Human Services (OKDHS) office. To find your local county OKDHS office, call 405-521-2779. Oklahoma Health Care Authority - You can print an application from the OHCA website at www.okhca. org. To request an application by mail, call 800-987-7767 or 711(TDD). Translation services are available. What happens after I apply? You will receive a letter telling you if you are approved or denied. First time members will receive a white plastic medical I.D. card, if approved. You must present the medical I.D. card before receiving services. If approved you may qualify for SoonerPlan for up to 12 months.
