We have expanded our program to 99 or fewer employees and added college students. The new
income guidelines are also in effect. Applications have been accepted beginning March 1, 2009.
Employer: Employers are automatically renewed every year. If there are any
changes with employer contact information, added staff, qualified health plan, or
terminated staff please fill out the Small Business Change Form (O-EPIC-4) and fax it in
Employees: Once, the employers have been automatically renewed in our system
the employees/individuals will need to reapply every year.
On the ESI plan, the spouse may be qualified if they work 24 hours per week for any
size employer. If the spouse works 25+ hours, they must be working for a small
business with 99 or fewer employees.
The forms for the employers and employees/individuals have been made fillable on
the Insure Oklahoma website.
College Student Tab:
The college student tab has been added to the Insure Oklahoma home page with
a complete list of accredited college/universities.
1. With the ESI Student who wants to be covered under Mom and Dad's group policy through an
Employer - would they enroll using the Student Enrollment Icon or would they enroll as a dependent with
the rest of the family? As a dependent with the family
2) How does the ESI eligible student enroll for the subsidy if the parents are already enrolled under ESI?
If the student is on the application already they can use a change form if the student is not on the
application they will need to reapply.

We have a new design of ESI and IP brochures arriving this week with the updated
