One of the smelters was owned and operated by the Bartlesville Zinc Company (BZC). On behalf of the successor parent company of BZC, Cyprus Amax Minerals Company (Cyprus Amax) has been working with the Oklahoma Department of Environmental Quality (DEQ) to investigate and perform necessary remediation at what is now known as the Collinsville Smelter site. The second zinc smelter is the Tulsa Fuel & Manufacturing (Tulsa Fuel) site, which is being addressed by DEQ and the U.S. Environmental Protection Agency (EPA). These agencies have completed their investigation of the Tulsa Fuel site, and remediation of the site is anticipated to occur over the next few years (DEQ, 2008). Cyprus Amax is not associated with the Tulsa Fuel site and the entities potentially liable for environmental conditions at this site have declined to participate. Due to the close proximity of the two former smelter sites to the town of Collinsville, the agencies believe that past smelter operations could have resulted in the presence of metals within Collinsville soils at concentrations higher than what would be typically found in this area. Typical metals associated with zinc smelter operations include arsenic, cadmium, and lead. Potential smelter-related sources of these metals within Collinsville include historic air emission deposition and use of smelter materials as yard fill or for construction projects. To determine whether soil within Collinsville contain elevated concentrations of these metals, the agencies’ investigation of the Tulsa Fuel site included soil sampling at approximately 200 residential properties in 2007. The sampling results indicated that soil at 10 of these properties contained concentrations of lead, arsenic, or cadmium above the cleanup levels established by the agencies for Collinsville residential properties (DEQ, 2008). Based upon the results of this initial town site investigation, DEQ has requested that Cyprus Amax conduct a more comprehensive soil investigation and cleanup program in and around Collinsville. Pursuant to a 2009 Consent Decree with DEQ, Cyprus Amax initiated the Collinsville Soil Program (CSP) in January 2009, which provides all Collinsville property owners the opportunity to have soil on their properties tested and if necessary, remediated. In addition, DEQ also requested that Cyprus Amax expedite soil remediation at the 10 properties identified as exceeding DEQ cleanup levels during the 2007 investigation of the Tulsa Fuel site. This work was conducted in 2010 pursuant to an Interim Remedial Action Work Plan (IRAWP) (Shaw, 2009a) approved by DEQ. This Interim Remedial Action Completion Report (IRACR) describes the construction activities associated with addressing these properties. INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 2 APRIL 2011 2.0 INTERIM REMEDIAL ACTION PROGRAM Provided below is a listing of nine of the 10 properties initially identified by DEQ for interim remedial action (IRA). Also included in this list is the 20-acre City Park (Parcel Property Identification Number [PPIN] 8509), which Cyprus Amax subsequently agreed to include in the IRA program in order to facilitate the City’s schedule for park renovation. Absent from this list is the parcel consisting of PPINs 3568, 3571, 3573, and 3575. This parcel is part of the Collinsville Smelter site and contains a significant amount of smelter waste. Accordingly, this parcel will be addressed as part of the remediation plan for the Collinsville Smelter site rather than part of the Collinsville Soil Program. The two additional properties are listed below. PPIN Address 2359 427 S 12th Street W 13534 921 W Oak Street N The locations of the properties included in the IRA are provided in Figure 1. Property owners were contacted via mail and in-person visits to review sampling results and to obtain access to the property for pre-confirmation sampling. Pre-confirmation sampling, which included characterizing all yard areas within a property and determining the final removal depth, was performed at each property as outlined in Sections 3.1.2 through 3.1.5 of the IRAWP. Prior INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 3 APRIL 2011 to conducting pre-confirmation sampling at the properties, the sampling teams conducted inspections of the properties for visible smelter material as outlined in Section 3.1.6 of the IRAWP. Table 1 presents the EPA sampling data and the CSP sampling data by yard area for each property. In cases where the CSP sampling data for a yard area indicated the yard area exceeded the DEQ Residential Soil Cleanup Levels (Cleanup Levels), but the EPA sampling data for the same yard area did not exceed (or vice versa), Cyprus Amax considered the yard area to exceed the Cleanup Levels. Cyprus Amax contracted with Environmental Restoration, LLC (ER) as the remediation contractor to establish and maintain the Temporary Staging Area and to perform remediation and restoration activities at the IRA properties. Cyprus Amax also contracted Shaw Environmental & Infrastructure, Inc. (Shaw) to conduct oversight and document the interim remediation work to ensure the work was completed in accordance to the conditions set forth in the Consent Decree and IRAWP. At each of the properties, cleanup activities were performed within those yard areas where the arsenic, cadmium, or lead concentrations were shown to exceed the residential soil cleanup levels developed for Collinsville. These soil cleanup levels were adopted from the Tulsa Fuel Site Record of Decision (EPA, 2008) and are as follows: DEQ Residential Soil Cleanup Levels for the CSP Parameter Cleanup Level Arsenic 37 milligrams per kilogram (mg/kg) Cadmium 75 mg/kg Lead 500 mg/kg In general, soil was removed from those yard areas to a depth where the sampling results indicated that the average concentrations of arsenic, cadmium, or lead were below the CSP Cleanup Levels. In addition to the soil removal within individual yard areas, the cleanup included the removal of visible smelter material (e.g., smelter brick, broken retort, etc.) from the property. Excavations were backfilled with clean soil and restored to match surrounding conditions (e.g., sod for a yard or gravel for a driveway). Backfill soil was transported directly from the borrow source area (Figure 2) to the excavated yard area. The backfill soil source was investigated in-situ in December 2009 as outlined in the Borrow Source Sampling Plan (Shaw, 2009c) which is included in Appendix G along with the analytical results. The borrow source (Robert Meyers Dirt Pit) and Temporary Staging Area were approved by DEQ. Additional periodic sampling and analyses of clean material sources was conducted during remediation activities to verify INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 4 APRIL 2011 arsenic, cadmium, and lead concentrations were below the standards established by DEQ for backfill for the Collinsville Soil Program (Shaw, 2009b). The IRA activities began on March 17, 2010 with the setup of the Temporary Staging Area (Figures 2 and 3). Removal work began on March 24, 2010, and was completed at each of the 12 properties by June 11, 2010. The remediation activities at each of the properties are outlined in Figures 4 through 15. Table 2 summarizes the order the work was completed as well as the details related to the soil removal and visible smelter material removal information for each property. A photo log documenting a portion of sampling, remediation, and restoration activities is included in Appendix F. Photos were collected during each phase of the work for each yard area during the project. Dust monitoring at the properties and the Temporary Staging Area was performed in accordance with Appendix E of the IRAWP. Air monitoring action levels are shown in the table below. Dust suppression equipment generally consisted of standard garden hoses with spray regulators and misters. Air monitoring data was collected at the Temporary Staging Area and each cleanup property, and is presented graphically in Appendix A. As shown, there were a few exceedances of dust monitoring limits at properties and at the Temporary Staging Area. On occasion background dust not directly related to cleanup activities from upwind contributed to an exceedance. For other exceedances, actions consisted of dust suppression and work stoppages on very windy days. Air Monitoring Action Levels Condition Action Soil Particulate, 5 minute average x < 100 μg/m3 100 μg/m3< x < 120 μg/m3 > 120 μg/m3 No additional dust control is required Active dust control measures begun, continuous observation of RAM until averages are below 100μg/m3 Work stoppage until additional dust mitigation measures implemented Construction quality assurance/quality control (QA/QC) testing and inspection procedures were implemented in accordance with the Quality Assurance Project Plan (QAPP) to ensure proper construction and compliance with the cleanup construction plans and procedures. Once the Construction Contractor (CC) achieved the targeted excavation depth(s), the Construction Project Coordinator (CPC) confirmed the excavation depth using methods outlined in the QAPP. Backfill of the excavations followed as soon as practical in order to minimize the amount of time the excavation areas were left open by the CC. The excavated areas were backfilled with clean INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 5 APRIL 2011 materials to restore pre-excavation grades and modified as necessary to facilitate proper drainage. Excavated driveways, parking areas, and other areas subject to vehicular traffic were replaced with compacted clean soil and a minimum of 4 inches of clean gravel top surfacing. The CC prepared a construction Health and Safety Plan (HASP) and maintained a designated Health and Safety Coordinator for the duration of the project. The Health and Safety Coordinator ensured all CC personnel conformed to the HASP requirements. The CC worked approximately 960 man-hours without an injury or incident during the interim remediation work. In total, the IRA work included excavating approximately 3,860 cubic yards of soil from 12 properties. Additionally, approximately 150 cubic yards of visible smelter material and associated soil were excavated from 5 of the properties. INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 6 APRIL 2011 3.0 CHARACTERIZATION AND DISPOSAL OF EXCAVATED SOIL Excavated soil, and other materials, removed during the IRA were transported to a separate property leased from the City of Collinsville by Cyprus Amax (Figure 2), where the material was temporarily stockpiled, sampled, and analyzed to characterize the material prior to final shipment to a permitted disposal facility. The two-acre Temporary Staging Area (Figure 2) was located just north of Collinsville, but within the CSP Study Area. Prior to construction of the Temporary Staging Area, surface soils in the area were sampled for arsenic, cadmium, and lead using analytical methods (EPA SW 846 methods for Resource Conservation and Recovery Act [RCRA] metals EPA 6010B) specified in the QAPP for the CSP (Shaw, 2009b). In addition, samples were analyzed for total petroleum hydrocarbons (TPH) in the diesel range organics (DRO) and gasoline range organics (GRO). A sub-grade buffer was constructed consisting of polyethylene sheeting overlain with a minimum 12-inch layer of compacted soil from the local borrow source. A 6-foot tall chain link fence with a locking access gate was established around the perimeter of the Temporary Staging Area to provide security against unauthorized access. A stormwater run-on/run-off diversion berm was also constructed around the perimeter of the Temporary Staging Area. Stormwater inspections, maintenance, and repairs at the Temporary Staging Area were performed in accordance with Appendix C of the IRAWP. Copies of the weekly stormwater inspection logs are included in Appendix B. Excavated soil characterization at the Temporary Staging Area included analysis for lead, arsenic, and cadmium using the Toxicity Characteristic Leaching Procedure (TCLP) to determine if the material exceeded the regulatory threshold for the RCRA characteristic for hazardous waste. Samples were collected at a rate of one composite sample (5-point composite) per stockpile (200 cubic yards), which was acceptable to DEQ and the final disposal facility. Samples were also tested for other analytes, as necessary, to qualify the material as acceptable for disposal at the Republic Waste landfill in Stillwater, Oklahoma. The stockpile sampling results are presented in Appendix C. These results indicate that none of the soil stockpiles exceeded the TCLP criteria. HRS Trucking was contracted by Republic Waste for the waste transportation. The manifest summary as well as copies of the manifests are presented in Appendix D. Upon completion of the IRA, the Temporary Staging Area was returned to its pre-construction condition, which included the removal of all excavated soil stockpiles, removal of the soil working pad, removal of the diversion berms, and removal of the security fence. Once the working pad and all stockpiled soils were removed, the footprint of the staging area was sampled and analyzed using the same approach that was used prior to the commencement of construction activities as described previously in this section. Appendix E INTERIM REMEDIAL ACTION COMPLETION REPORT COLLINSVILLE SOIL PROGRAM 7 APRIL 2011 presents the pre- and post-construction sampling data and figures. Arsenic, cadmium and lead concentrations as shown in Table E-1 do not exceed the DEQ residential soil cleanup levels for the CSP. As the location of the Temporary Staging Area was a former industrial site, the analytical results for TPH were compared to the DEQ industrial standard of 500 mg/kg. As shown in Table E-2 of Appendix E, all analytical results for TPH were below the industrial standard. Final restoration of the Temporary Staging Area included placement of a 6-inch layer of topsoil for additional cover material and reseeding of the topsoil layer with native grasses. The Temporary Staging Area was closed October 15, 2010. Restoration activities were approved by the City of Collinsville prior to the remediation contractor’s demobilization from the site. 1.0 BACKGROUND
In the early 1900s, two zinc smelters operated for several years about 1 mile south of
Collinsville, Oklahoma. One of the smelters was owned and operated by the Bartlesville Zinc
Company (BZC). On behalf of the successor parent company of BZC, Cyprus Amax Minerals
Company (Cyprus Amax) has been working with the Oklahoma Department of Environmental
Quality (DEQ) to investigate and perform necessary remediation at what is now known as the
Collinsville Smelter site.
The second zinc smelter is the Tulsa Fuel & Manufacturing (Tulsa Fuel) site, which is being
addressed by DEQ and the U.S. Environmental Protection Agency (EPA). These agencies have
completed their investigation of the Tulsa Fuel site, and remediation of the site is anticipated to
occur over the next few years (DEQ, 2008). Cyprus Amax is not associated with the Tulsa Fuel
site and the entities potentially liable for environmental conditions at this site have declined to
participate.
Due to the close proximity of the two former smelter sites to the town of Collinsville, the
agencies believe that past smelter operations could have resulted in the presence of metals within
Collinsville soils at concentrations higher than what would be typically found in this area.
Typical metals associated with zinc smelter operations include arsenic, cadmium, and lead.
Potential smelter-related sources of these metals within Collinsville include historic air emission
deposition and use of smelter materials as yard fill or for construction projects.
To determine whether soil within Collinsville contain elevated concentrations of these metals,
the agencies’ investigation of the Tulsa Fuel site included soil sampling at approximately 200
residential properties in 2007. The sampling results indicated that soil at 10 of these properties
contained concentrations of lead, arsenic, or cadmium above the cleanup levels established by
the agencies for Collinsville residential properties (DEQ, 2008).
Based upon the results of this initial town site investigation, DEQ has requested that Cyprus
Amax conduct a more comprehensive soil investigation and cleanup program in and around
Collinsville. Pursuant to a 2009 Consent Decree with DEQ, Cyprus Amax initiated the
Collinsville Soil Program (CSP) in January 2009, which provides all Collinsville property
owners the opportunity to have soil on their properties tested and if necessary, remediated.
In addition, DEQ also requested that Cyprus Amax expedite soil remediation at the 10 properties
identified as exceeding DEQ cleanup levels during the 2007 investigation of the Tulsa Fuel site.
This work was conducted in 2010 pursuant to an Interim Remedial Action Work Plan (IRAWP)
(Shaw, 2009a) approved by DEQ. This Interim Remedial Action Completion Report (IRACR)
describes the construction activities associated with addressing these properties.

2.0 INTERIM REMEDIAL ACTION PROGRAM
Provided below is a listing of nine of the 10 properties initially identified by DEQ for interim
remedial action (IRA). Also included in this list is the 20-acre City Park (Parcel Property
Identification Number [PPIN] 8509), which Cyprus Amax subsequently agreed to include in the
IRA program in order to facilitate the City’s schedule for park renovation. Absent from this list
is the parcel consisting of PPINs 3568, 3571, 3573, and 3575. This parcel is part of the
Collinsville Smelter site and contains a significant amount of smelter waste. Accordingly, this
parcel will be addressed as part of the remediation plan for the Collinsville Smelter site rather
than part of the Collinsville Soil Program.

At the request of DEQ, two child daycare properties with soil exceedances in one or more yard
areas, which were sampled during the CSP activities, were added to the scope of the IRA. The
two additional properties are listed below.

The locations of the properties included in the IRA are provided in Figure 1.
Property owners were contacted via mail and in-person visits to review sampling results and to
obtain access to the property for pre-confirmation sampling. Pre-confirmation sampling, which
included characterizing all yard areas within a property and determining the final removal depth,
was performed at each property as outlined in Sections 3.1.2 through 3.1.5 of the IRAWP. Prior

to conducting pre-confirmation sampling at the properties, the sampling teams conducted
inspections of the properties for visible smelter material as outlined in Section 3.1.6 of the
IRAWP. Table 1 presents the EPA sampling data and the CSP sampling data by yard area for
each property. In cases where the CSP sampling data for a yard area indicated the yard area
exceeded the DEQ Residential Soil Cleanup Levels (Cleanup Levels), but the EPA sampling data
for the same yard area did not exceed (or vice versa), Cyprus Amax considered the yard area to
exceed the Cleanup Levels.
Cyprus Amax contracted with Environmental Restoration, LLC (ER) as the remediation
contractor to establish and maintain the Temporary Staging Area and to perform remediation and
restoration activities at the IRA properties. Cyprus Amax also contracted Shaw Environmental
& Infrastructure, Inc. (Shaw) to conduct oversight and document the interim remediation work to
ensure the work was completed in accordance to the conditions set forth in the Consent Decree
and IRAWP.
At each of the properties, cleanup activities were performed within those yard areas where the
arsenic, cadmium, or lead concentrations were shown to exceed the residential soil cleanup
levels developed for Collinsville. These soil cleanup levels were adopted from the Tulsa Fuel
Site Record of Decision (EPA, 2008) and are as follows:

In general, soil was removed from those yard areas to a depth where the sampling results
indicated that the average concentrations of arsenic, cadmium, or lead were below the CSP
Cleanup Levels. In addition to the soil removal within individual yard areas, the cleanup
included the removal of visible smelter material (e.g., smelter brick, broken retort, etc.) from the
property.
Excavations were backfilled with clean soil and restored to match surrounding conditions (e.g.,
sod for a yard or gravel for a driveway). Backfill soil was transported directly from the borrow
source area (Figure 2) to the excavated yard area. The backfill soil source was investigated in-situ
in December 2009 as outlined in the Borrow Source Sampling Plan (Shaw, 2009c) which is
included in Appendix G along with the analytical results. The borrow source (Robert Meyers
Dirt Pit) and Temporary Staging Area were approved by DEQ. Additional periodic sampling
and analyses of clean material sources was conducted during remediation activities to verify

arsenic, cadmium, and lead concentrations were below the standards established by DEQ for
backfill for the Collinsville Soil Program (Shaw, 2009b).
The IRA activities began on March 17, 2010 with the setup of the Temporary Staging Area
(Figures 2 and 3). Removal work began on March 24, 2010, and was completed at each of the
12 properties by June 11, 2010. The remediation activities at each of the properties are outlined
in Figures 4 through 15. Table 2 summarizes the order the work was completed as well as the
details related to the soil removal and visible smelter material removal information for each
property. A photo log documenting a portion of sampling, remediation, and restoration activities
is included in Appendix F. Photos were collected during each phase of the work for each yard
area during the project.
Dust monitoring at the properties and the Temporary Staging Area was performed in accordance
with Appendix E of the IRAWP. Air monitoring action levels are shown in the table below.
Dust suppression equipment generally consisted of standard garden hoses with spray regulators
and misters. Air monitoring data was collected at the Temporary Staging Area and each cleanup
property, and is presented graphically in Appendix A. As shown, there were a few exceedances
of dust monitoring limits at properties and at the Temporary Staging Area. On occasion
background dust not directly related to cleanup activities from upwind contributed to an
exceedance. For other exceedances, actions consisted of dust suppression and work stoppages
on very windy days.

Construction quality assurance/quality control (QA/QC) testing and inspection procedures were
implemented in accordance with the Quality Assurance Project Plan (QAPP) to ensure proper
construction and compliance with the cleanup construction plans and procedures. Once the
Construction Contractor (CC) achieved the targeted excavation depth(s), the Construction
Project Coordinator (CPC) confirmed the excavation depth using methods outlined in the QAPP.
Backfill of the excavations followed as soon as practical in order to minimize the amount of time
the excavation areas were left open by the CC. The excavated areas were backfilled with clean

materials to restore pre-excavation grades and modified as necessary to facilitate proper
drainage. Excavated driveways, parking areas, and other areas subject to vehicular traffic were
replaced with compacted clean soil and a minimum of 4 inches of clean gravel top surfacing.
The CC prepared a construction Health and Safety Plan (HASP) and maintained a designated
Health and Safety Coordinator for the duration of the project. The Health and Safety Coordinator
ensured all CC personnel conformed to the HASP requirements. The CC worked approximately
960 man-hours without an injury or incident during the interim remediation work.
In total, the IRA work included excavating approximately 3,860 cubic yards of soil from 12
properties. Additionally, approximately 150 cubic yards of visible smelter material and
associated soil were excavated from 5 of the properties.

3.0 CHARACTERIZATION AND DISPOSAL OF EXCAVATED
SOIL
Excavated soil, and other materials, removed during the IRA were transported to a separate
property leased from the City of Collinsville by Cyprus Amax (Figure 2), where the material
was temporarily stockpiled, sampled, and analyzed to characterize the material prior to final
shipment to a permitted disposal facility. The two-acre Temporary Staging Area (Figure 2) was
located just north of Collinsville, but within the CSP Study Area.
Prior to construction of the Temporary Staging Area, surface soils in the area were sampled for
arsenic, cadmium, and lead using analytical methods (EPA SW 846 methods for Resource
Conservation and Recovery Act [RCRA] metals EPA 6010B) specified in the QAPP for the CSP
(Shaw, 2009b). In addition, samples were analyzed for total petroleum hydrocarbons (TPH) in
the diesel range organics (DRO) and gasoline range organics (GRO). A sub-grade buffer was
constructed consisting of polyethylene sheeting overlain with a minimum 12-inch layer of
compacted soil from the local borrow source. A 6-foot tall chain link fence with a locking access
gate was established around the perimeter of the Temporary Staging Area to provide security
against unauthorized access. A stormwater run-on/run-off diversion berm was also constructed
around the perimeter of the Temporary Staging Area. Stormwater inspections, maintenance, and
repairs at the Temporary Staging Area were performed in accordance with Appendix C of the
IRAWP. Copies of the weekly stormwater inspection logs are included in Appendix B.
Excavated soil characterization at the Temporary Staging Area included analysis for lead,
arsenic, and cadmium using the Toxicity Characteristic Leaching Procedure (TCLP) to determine
if the material exceeded the regulatory threshold for the RCRA characteristic for hazardous
waste. Samples were collected at a rate of one composite sample (5-point composite) per
stockpile (200 cubic yards), which was acceptable to DEQ and the final disposal facility.
Samples were also tested for other analytes, as necessary, to qualify the material as acceptable
for disposal at the Republic Waste landfill in Stillwater, Oklahoma.
The stockpile sampling results are presented in Appendix C. These results indicate that none of
the soil stockpiles exceeded the TCLP criteria. HRS Trucking was contracted by Republic Waste
for the waste transportation. The manifest summary as well as copies of the manifests are
presented in Appendix D. Upon completion of the IRA, the Temporary Staging Area was
returned to its pre-construction condition, which included the removal of all excavated soil
stockpiles, removal of the soil working pad, removal of the diversion berms, and removal of the
security fence. Once the working pad and all stockpiled soils were removed, the footprint of the
staging area was sampled and analyzed using the same approach that was used prior to the
commencement of construction activities as described previously in this section. Appendix E

presents the pre- and post-construction sampling data and figures. Arsenic, cadmium and lead
concentrations as shown in Table E-1 do not exceed the DEQ residential soil cleanup levels for
the CSP. As the location of the Temporary Staging Area was a former industrial site, the
analytical results for TPH were compared to the DEQ industrial standard of 500 mg/kg. As
shown in Table E-2 of Appendix E, all analytical results for TPH were below the industrial
standard.
Final restoration of the Temporary Staging Area included placement of a 6-inch layer of topsoil
for additional cover material and reseeding of the topsoil layer with native grasses. The
Temporary Staging Area was closed October 15, 2010. Restoration activities were approved by
the City of Collinsville prior to the remediation contractor’s demobilization from the site.
