Oklahoma’s Older Americans Act Title III Senior Nutrition Program General Purpose To provide comprehensive nutritional services to older persons through programs and services in five main categories: nutrition, health, social engagement, education and access to services. Services Available • Congregate meals served at a nutrition site • Home-delivered meals delivered to eligible homebound persons • Nutrition education information provided by a licensed/registered dietician (RD) • Nutrition counseling provided by RD to persons at high nutritional risk • Outreach services to help seniors identify service needs • Health promotion to provide evidence-based exercise, chronic disease self-management, diabetes self-management, screenings and disease prevention education • Medication management to reduce medication errors and adverse drug reactions • Transportation available at some sites Nutrition Sites • 227 Nutrition sites in State Fiscal Year (SFY) 2011 were operated by 20 local service providers in 77 counties • Nutrition sites were the source of 1,821,353 congregate meals and 1,401,105 home-delivered meals served to senior Oklahomans in SFY 2010 Eligibility Requirements • Must be 60+ years of age or a spouse, regardless of age • There is no income criteria to participate • Everyone is given the opportunity to contribute • Guests pay full price of the meal Continued on back
